We welcome applications from people who live within our practice area. We may also consider applications from people who live outside the area.
How to register
You can apply using our online registration service or a paper form. Please contact us if you need help because of a communication, language, literacy, digital or accessibility need.
If you take regular medication, please make sure you have at least one month’s supply when registering, as it can take time for your medical records to transfer electronically.
We may ask for information to help us match your NHS record and provide safe care.
Registration is free.
You do not need proof of address, immigration status, identification or an NHS number to apply.
We will tell you if your application has been accepted and explain any service limitations.
If you live within our practice area
We will check that your home address is within our published practice boundary.
If you apply through the Health-tech online service, your patient information will already have been verified. Our registration team will review any query, discrepancy or safeguarding alert shown in the Health-tech portal and take appropriate action.
We will check for any immediate needs, such as repeat medicines, current treatment or an urgent appointment.
Once registered, we will explain how to access appointments, prescriptions, online services, urgent care and home visits.
Catchment area
Before filling out a registration form, check you are in our catchment area.
If you live outside our practice area
We may consider an out-of-area application if your home address is no more than 5 miles from the Practice.
Acceptance is not automatic. We will consider whether registering with us would be practical and safe for you.
If you apply through the Health-tech online service, your details will already have been verified. Our registration team will review any query, discrepancy or safeguarding alert shown in the portal.
We may ask about your ability to attend the surgery, your current care and whether you need home visits or local community services.
We will tell you whether your application has been accepted and explain any limitations before registration is completed.
When out-of-area registration may not be suitable
Out-of-area registration may not be suitable if you need, or are likely to need:
- Frequent home visits or complex care at home.
- Close support from community nursing, maternity, additional services such as mental health, safeguarding, rehabilitation or social care services near your home.
- Regular treatment that cannot reasonably be provided at the surgery.
- Care through local pathways that the Practice cannot access or coordinate safely.
We will consider your individual circumstances and any reasonable adjustments you may need.
IMPORTANT: Limited services for out-of-area patients
Please read this before applying: If we accept you as an out-of-area patient, we will not provide home visits at your home address. You must be able to attend the Practice for care. If you have an urgent medical need at home and cannot attend, contact NHS 111 or the local service available in your area. Call 999 in a life-threatening emergency.
- Community, district nursing, maternity, mental health, rehabilitation, social care and other local services are often organised according to your home address.
- Some referrals or Primary Care Network services may be unavailable, delayed or provided differently outside our local area.
- We cannot guarantee that an equivalent service will be available near your home or how quickly it will respond.
- You must keep your address and contact details up to date and attend the Practice when asked.
By choosing out-of-area registration, you confirm that you understand these limitations and that registering closer to home may give you easier access to local and home-based services.
If your circumstances change or you move out of area.
Please tell us if you change your address.
- You may be asked to register with a GP practice closer to your new home.
- You may be able to stay registered with us if your health needs have not changed.
- If you stay with us, you will be re-registered as an out-of-area patient.
Some services may be different or may not be available to out-of-area patients. We will send you a letter explaining the changes and which services will not be available before your registration changes.
Please tell us if your health needs change. You should also tell us about changes to your mobility, caring arrangements or the services you need. We may review whether it is still safe and suitable for you to remain registered as an out-of-area patient. If it is no longer suitable, we will explain why and help you understand how to register with a GP practice closer to your home.
If we cannot accept your application
If we cannot accept your application, we will explain the reason. You can ask the Practice Manager to review the decision or use our complaints procedure.
Fair access and privacy
We consider every application fairly, protect your personal and clinical information, and provide reasonable adjustments where needed.
