To update details like your name, address, phone numbers or email address:
- use your NHS account (through the NHS website or NHS App)
- use your Patient Access account
- fill out a change your personal details form
Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
If you are 16–18 (or about to turn 16), please update your contact details so they are your own, not a parent’s. You will have received a card from us — just scan the QR code to update your phone number and email address so we can contact you directly about appointments, results and messages. Thank you for helping us keep your record accurate.
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.
Changing your name
You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
