(Enquiries can only be submitted during core contracted hours 8am to 6:30pm Monday to Friday).
When completing this online form it is important that you understand how your query will be dealt with in the practice. Please take a moment to read this before completing your form.
Your enquiry will initially be viewed by one of our admin team. If they feel that they can deal with it directly then they will do so (eg a question about whether a result has been received or not). If the query is more of a clinical one, then this will be sent to a GP to review within a 24 hours period.
As GPs are viewing these queries in between clinics, they will not always be able to respond to you directly themselves. There will be different outcomes depending on the nature of your query. The most common will be one of the following –
- You need to book a routine GP/ANP appointment to discuss further. In this case we will send you a text message with a booking link which you can use to book an appointment.
- If your problem is deemed to be one which requires an urgent appointment, then the GP will ask our reception team to book you in one of our same day urgent GP appointments. However, please note that these are limited in number each day, and once these slots are full you will be advised to contact 111 or attend the out of hours centre. If you already know that you will need an urgent appointment then phoning reception first thing in the morning remains the best way to access this.
- You may need to book a blood test, nurse or pharmacist appointment – a text message with booking link would be sent to enable you to do this.
- If your query relates to medication then the GP may forward this to our pharmacist team. You will be advised of this by text and the pharmacist team will then contact you.
- If your query relates to a musculoskeletal problem you may be advised to book in with our MSK specialist team (FCP) for a more comprehensive assessment.
If you do have an urgent medical query you should telephone the surgery or contact the out of hours service by calling 111. In an emergency, please contact 999.

Severe difficulty breathing
not being able to get words out, choking or gasping
Heavy bleeding
spraying, pouring or enough to make a puddle
Severe injuries
after a serious accident
Seizure (fit)
shaking or jerking because of a fit, or unconscious (can’t be woken up)
Sudden, rapid swelling
of the lips, mouth, throat or tongue
Labour or childbirth
waters breaking, more frequent intense cramps (contractions), baby coming, or just born
British Sign Language (BSL) speakers can make a video call to 999.
Deaf people can use 18000 to contact 999 using text relay.
