In order to ensure a smooth transition from the medical practice you are leaving, please ensure you have already requested four weeks’ supply of any regular medication prior to leaving.
Catchment area
Before filling out a registration form, check you are in our catchment area.
The practice boundary area encompasses the villages and towns of New Mills, Hayfield, Birch Vale, Thornsett, Rowarth, Furness Vale, Buxworth and Chinley.
We accept patients from parts of Whaley Bridge and Disley.
However if you require the district nurse service or home visits they will only visit patients within some of the above areas and will not cross County boundaries.

Registering with the surgery
To register with the surgery:

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Please ensure that the digital registration form is fully completed, you will not be able to register without completing all mandatory information requirements.
To register as a new patient we welcome anyone living within our practice boundary. However, we allow out of area patients to register, but any out of area patients living outside the practice boundary must be aware that certain services or community care are unavailable.
All out of area registrations will be subject to GP approval in the interest of patient safety.
